Free and confidential family planning services for teens, women, and men.
Who it may help
To be eligible, you need to meet these criteria:
- You're a New York State resident.
- You're a U.S. Citizen, National, or lawfully present.
- You cannot be enrolled in Medicaid.
- If you have Medicaid, confidential family planning is already included in your coverage.
- Your income is equal to or less than these income limits:
- If your household size is 1, your yearly income must be at or below $2,909.
- If your household size is 2, your yearly income must be at or below $3,931.
- If your household size is 3, your yearly income must be at or below $4,953.
- If your household size is 4, your yearly income must be at or below $5,975.
- If your household size is 5, your yearly income must be at or below $6,997.
- If your household size is 6, your yearly income must be at or below $8,019.
- If your household size is 7, your yearly income must be at or below $9,041.
- If your household size is 8, your yearly income must be at or below $10,063.
- For each additional person, add:, add $1,023 to the limit.
What you may need
You will need documents to prove the following:
- Identity: such as a photo ID, drivers license, passport, or other official photo ID
- Age: such as Birth certificate, adoption or foster care records, or official hospital or school records
- Citizenship or immigration status: such as your birth certificate, US passport, or green card
- Social Security Number
- Income (if any): such as a recent pay stub, letter from employer stating income, unemployment, child support, or alimony award letter
- Residency: such as a photo ID with your address, postmarked envelope, utility bill, or lease
- Health insurance: such as benefit card or policy statements (if you have health insurance).
